How do you envision Family-Centered Care?

Comprehensive Coordinated Family-Centered Care in a Medical Home

Comprehensive Coordinated Care in Family-Centered Medical Home is of great value to the client, family and the community. Building a relationship with your doctor and medical staff is ideal for optimal health and wellness. A personal established relationship with a provider is important for those who suffer from an acute or chronic disease(s). For the reason being that, the provider can advocate for the client and coordinate his/her healthcare needs. Family-Centered Medical Homes focus on caring for the whole person’s needs, as well as the individuals in their family circle.Flexibility and accessibility to care are key reasons why clients want to receive their primary care in a Medical Home practice.

They often have flexible and expanded hours by staying open after 5:00 pm and are open up on
Saturdays. In addition, clients who have debilitating illnesses and are unable to make it into the office, a medical home can still help to meet their healthcare needs. Clients are able to have access to a patient portal in order to communicate with medical staff. In addition, medical home practices may offer telemedicine visits and make referrals for home visits. Medical Homes also have laboratory hours for blood work, on-site specialty services like Podiatry (Foot Doctor) and link clients to other resources. Furthermore, by providing accessibility to the client, this too can help reduce non-emergency visits to the hospital Emergency Room.

The holidays can be a stressful time trying to  buying gifts and all the planning that incurs.  Taking time to appreciate the things in life  such as your health and lifestyle choices really matters.  Medical studies show that even though a person may have a chronic medical condition, like Diabetes, Hypertension, Sickle Cell Disease etc., they can still improve their overall health outcomes.  People who participate in  comprehensive lifestyle modification programs experience clinical meaningful  improvements in their overall health & wellness.  That means, take a step forward   by scheduling  a yearly Wellness Exam for a  Preventative Maintenance check-up.  Radio Personality, Tom Joyner is an advocate for the “Take a Loved One to the    Doctor Day.”  Health Economists state that protecting yourself against illness and   disability is one of the most important financial investments you can make during your life.  When problems are addressed in a timely manner, long-term  health  outcomes improve drastically.   To find out more information, go to the Cleveland     Clinic Center for Lifestyle Medicine @ www.clevelandclinic.org/wellness.

Keys to a Happier Healthier Life

Photo:  theodysseyonline.com
More people are exploring ways to find inner peace and well-being in these uncertain times.  Deepak Chopra, MD co-founder of the Center for Wellbeing and a world pioneer of integrative medicine defines Happiness as a state of well-being, joy and ease when you seem to be in the flow and things just happen. People who are Happy are more likely to be healthier, have      better relationships, do good/meaningful work and be successful at it.  When you get down to it, most people generally want peace, harmony, laughter and love which is a personal state of well-being.  Professor Jack Shonkoff, Professor at Harvard Medical School found that negative emotions can harm the body by altering the biological system that wears and tears on the body.  People who have chronic anger and anxiety can increase their risk of cardiac (heart) problems and trigger stress hormones that increase inflammation in the body.

For those who are in a difficult place right now, both experts agree that change has to start with you.  When you are Happy, you tend to exert more positive energy.  That means when one person sees a problem-another sees the problem as an opportunity.  Practicing meditation and also working with a therapist can help one re-channel negative thoughts, feelings and emotions.  Experts also suggest that there are Keys to a Happier and Healthier life.  Here are few tips to help one take strides towards a better emotional balance:  live in the moment, find resilience, manage your energy, treat yourself well    sometime, venture outside your comfort zone and show compassion outwardly.  Start by making an appointment with your primary care provider to discuss your concerns and let your provider help coordinate your care.

TOVA Sickle Cell Awareness Walk/Run hook-up joined two participants in marriage Saturday, November 11th

Congratulations to Dr. Raymisha  and Theo Jr. who met at the 2015 James Faucett III Sickle Cell Awareness run at Brandywine Park.  Even though Theo tried to keep up with Raymisha, he finally conquered by getting the hook up which led to a friendship beyond compare.
Our Wilmington Delaware community longed for a wedding of elegance even just to dress up and connect with family andfriends.  All of the above was achieved on Saturday, November 11th at Hanover Presbyterian Church and the receptions.  Tears of Joy and Best Wishes to you both.

A Consumer/Patient perspective from the Delaware Hospital Association's Conference on October 31st

I thought it would be valuable for the DHAs' stakeholders to hear the perspective from a consumer who suffers with Complex Chronic Conditions- Sickle Cell Disease, Asthma, Degenerative Disc Disease and Chronic Pain and utilizes the hospital system for sickle cell crisis and other complications.  By the way, this patient has 'private' insurance.

Re: "The Delaware Healthcare Association Conference, which was held on October 31, 2017, was an interesting and enlightening day session of information.  It consistently highlighted the positive and negative aspects of an already volatile relationship between the federal government, state governments, healthcare providers/professional across the U.S., and medical patients.  This was shown through the presentation of many different legislative bills, which affect the healthcare industry.  The presented bills have either been passed, thrown out, or tabled.

On another positive note, the DHA Conference was a great platform for many different businesses.  All of these different companies, that were at the conference, were both in the healthcare field and non-healthcare professions.  The conference allowed them to advertise, sponsor, and co-mingle all other businesses and participants at the DHA Conference.  These allowed for all the businesses to bring awareness their cause, to network, and offer their services to people who may have never known or had access to their business.  Overall, the conference was a success in providing a networking platform, healthcare education, and possible new services to many different people."
The Delaware Healthcare Association was formed in 1967 to assist Delaware hospitals in working on issues concerning health care and the hospital industry.  The primary role of the Association is to serve as a leader in the promotion of effective change in health services through collaboration and consensus building on health care issues at the State and Federal levels.  Delaware Hospital Association
Despite the challenges from the hospital systems and regulatory bodies, it is clear that healthcare costs are rising steadily and are unsustainable.  It is my hope that public policy, bureaucrats and healthcare administrators will put on the table the client/patient and value of a highly skilled and trained healthcare workforce.

Message from Dr. Nina: Nothing is Certain but Change

The anxiety and the normal doubt from the uncertainty one experiences when dealing with change is to be expected. Over the years in healthcare, technological advances along with Electronic Medical Records revolutionized care in a way no-one could ever imagine. In addition to TOVA moving to our new location at Family Medicine at Greenhill from Henrietta Johnson has been a challenge but surprisingly greater opportunities have presented itself.

Dr. Edward Creagan from the Mayo Clinic stated that we can not keep change from happening but we can choose how we react and respond. In other words, we can invest our energy feeling sorry for ourselves or figure how to adapt and thrive. Oftentimes, there is a sense of a loss of control over a situation that we cant keep from happening. A few tips that have helped me along this journey are: replace fearful thoughts with positive thoughts, regular exercise can help you stay calm to make clear and rational decisions and lastly keep on grinding. Yes, change can be frightening. But the right attitude, outlook and actions can help you find opportunities within that change.
                                                                     All the Best,

Sickle Cell is becoming a 'Sexy Chronic Disease' - $$

Just returning from a Sickle Cell conference at the National Institute of Health in Bethesda, MD and am awl of the number of new clinical trials in the pipeline to develop new medicines to treat and cure Sickle Cell Disease.  Over the years, I witnessed having just a few medications to treat the horrible complications persons experience from the damage causes from the 'sickling' of the
blood cells such as lung damage, heart enlargement and damage, kidney damage, joint and bone damage and etc.
The FDA recently approved new Drug called, Endari  aimed to reduce sickling of the red blood cells.  Although we are waiting for access to this medication, there is real hope on the Horizon.  There are also new Point of Care testing trials to rapidly diagnose people with sickle cell and trait and even in non-invasive 'no needle' stick methods.

To find out more about these new therapies, visit a
